The Food Facility Operator shall be the Person in Charge or shall designate a Person in Charge and shall ensure that a Person in Charge is present at the food facility during all hours of operation. The Person in Charge did not demonstrate adequate knowledge of the PA Food Code as evidenced by incorrect responses to food safety questions. Expired baby formula on site...and short dated (5/25) on shelf.... discarded. Raw eggs were stored above ready to eat foods in the deli cold hold. Time in lieu of temperature being used in the food facility to control ready to eat potentially hazardous foods without written procedures or documentation to verify disposition of food (Empanadas). Temperature measuring device for ensuring proper temperature of equipment is not available or readily accessible in equipment. Hood system is not adequate to remove heat and/or grease, as evidenced by condensate and/or grease collecting on the walls, ceilings, and fixtures. Non-food contact surfaces not cleaned at a frequency to preclude accumulation of dirt and soil. The handwash sink located in the bathroom does not have water at a temperature of at least 100 degrees F. Lights are not working in hood area. Improper locks and no door knob on bathroom door. No sign or poster posted at the handwash sink in the bathroom to remind food employees to wash their hands. Toilet seat broken Cleaning needed.

The Person in Charge did not demonstrate adequate knowledge of food safety in this food facility as evidenced by this non-compliant inspection. In-house packaged beef and goat not labeled with the name of the product. Product was observed in the upright refrigerated unit in the front of the salesfloor and in two rolling freezers in front of the meat service counter. Observed the three-compartment sink in the meat prep area not easily accessible due to equipment being stored in front of the unit. Observed knives stored on the magnetic knife holder in the meat prep area with visible food residue and were otherwise unclean to sight and touch. Observed the ice machine with a visible black moldlike substance on the interior as well as old food residue and debris on the interior lid and gaskets. Knives and cutting boards are not being cleaned before using with different types of raw animal-derived foods. Observed knives and utensils used to cut and process fish, pork and beef without being cleaned between species. Observed utensils used to process fresh goat being used to descale/process fresh whole fish in the meat prep area. Educated operator on proper utensil usage/cleaning and sanitizing between animal species. Non-food contact surfaces not cleaned at a frequency to preclude accumulation of dirt and soil. Observed the following: Heavy animal residue on the interior of the service meat case; Animal residue on the sliding glass doors, door channels and top of the service meat case. Old food residue, dishes and utensils observed in the handwash sink, indicating uses other than handwashing. Paper towel dispenser observed empty at the handwash sink in the meat prep area. No sign or poster posted at the handwash sink in the meat prep area to remind food employees to wash their hands. Broken and unused equipment observed being stored in the rear exterior of the facility and needs removed. Meat prep area of the food facility is extremely dirty, dusty, and in need of cleaning. Observed old food residue, dirt and animal processing residue on the floor, walls and ceiling of the meat prep area.

Knife disposed of/corrected on-site. Observed the following: Can opener located on the rear food prep table in the food prep area with a food residue on the blade; Soda gun nozzle behind the bar service area with a build-up of old syrup residue Corrected on site. Observed bottom shelves of the prep tables in the food prep area being covered in aluminum foil which had visible food residue and grease. Floor of the walk-in cooler in the food prep area observed with a damaged floor that is not easily cleanable. Observed the paper towel dispenser empty behind the bar serving area. Corrected on-site.
